SNOTY Submission Procedures
PROCEDURES FOR SUBMISSION OF APPLICATION FOR ALL CATEGORIES
A - Information must be submitted electronically. Curriculum vitae, supporting letters, and narrative must be scanned and uploaded . It is recommended to gather all required documents and submit at same time so you will be sure to include everything. If you have any questions, email snoty@gasn.org.
B- Fill out Nomination Form and Submit. You will receive and email with instructions on how and where to upload Nomination Package.
C- Include in the nomination package submission:
- Curriculum Vitae signed by nominee.
- Narrative describing the nominee's contributions in each of the categoreis named in the CRITERIA FOR SELECTION. Narrative must be signed by nominee.
- Supporting letters of recommendation:
- Maximum of four (4) letters, each limited to one page
- Letters may be from School Nurse colleagues, administrators, supervisor, teachers, parents, students or others. Letters should describe specific issues or topics related to nominee's qualification for the award. Use Scope and Standards of Professional School Nursing Practice (copyright 2001, National Association of School Nurses and American Nurses Association), and American Nurses Association Scope and Standards for Nurse Administrators, 2nd edition (copyright 2004) as references.
- Letters should directly reflect areas of rating that writer wishes to emphasize
- Local and affiliate acronyms must be spelled out when used for the first time
D- The total package is not to exceed 20 pages.
